Establishing a consistent bedtime routine is important in dealing with bedwetting. Having a routine helps to ensure that antidiuretic hormone (ADH)- which helps to regulate the amount of urine produced at night - is consistently being made at adequate amounts daily.

Some obvious recommendations like going to bed at the same time of night seem easy, but can be difficult to be consistent with busy kids and family.

Meditation just for 5-10 minutes per night can help kids wind down at night. I use the “Smiling Minds” app for my family. My kids actually look forward to it at bedtime.

Keep a set a sheets and PJs by the bedside for easy changing when an episode of bedwetting occurs.

Bedwetting alarms are helpful for some. But I didn’t put it on this list because it is not the right answer for all.

A bulking food does just that - help to bulk up the stool which makes it bigger and help to absorb more fluid. This can make the stool easier to pass. The only downside with bulling agents is that it requires plenty of fluids otherwise it can make the stool too hard to pass.

COVID has been shown to cause urinary symptoms like frequency, urgency, dysuria/pain and nocturia (waking up at night to pee). It can occur during and after the active COVID infection.

Recently at the American Urologic Association conference, some research was presented that showed more patients being discharged from the hospital after a COVID infection with these symptoms.

It appears that instead of an overactive bladder, there is more of an increased inflammation of the bladder after the infection. Treatment options possibly can include anti-inflammatories or steroids. But more studies need to be completed to determine average course of the symptoms and treatment effectiveness.

This is especially true if the antibiotics are causing side effects, significant resistance patterns or colitis.

PTNS is a therapy using electric stimulation of the posterior tibial nerve. Over time, there is improvement in bladder function. There is a lot of data showing improvement in urinary urgency/frequency. New data suggests there is also improvement in bedwetting and constipation.

Most children that start PTNS show improvement after several weeks and continue treatment for at least 3 months.

Kids and adults often find it embarrassing to talk about their pee and poop. Why? Everyone pees and poops! There should be open discussion about normal bodily functions.

Kids who are ashamed about talking about their poop and pee, have a much more difficult time resolving their constipation/encopresis and/or urinary incontinence.

Those who are open about talking about it with their medical provider and caretakers, will have a much easier time to resolve their pee and poop problem.

There is a lot of back and forth with adjustment of medications and supplements that usually occurs. Often I suggest a “no flush rule” so the caretaker can also understand what the bowel movements are like.

Let’s start the pee and poop discussion young! There are many books about pooping.
The classic “Everyone Poops” by Taro Gomi is a good place to start.

🧫D-Mannose is the sugar in cranberries that specifically targets the bladder lining to prevent bacterial growth in the bladder. It works differently than antibiotics. It also will not cause antibiotic resistance.

💊D-Mannose comes in pill and powder forms (as well has hard to find liquid). When taken daily, it can help prevent UTIs.

⭐️For adults, 2000 mg daily
⭐️For children, 1000 mg daily

It cannot be used instead of antibiotics to treat an active UTI. But it is a good alternative to prophylactic antibiotics.

The TikTok trend, devious licks, has made the news (🔗in bio). Unfortunately, for many of my patients, the bathrooms have been closed throughout the school day. This leads to urine holding, and many of my patients are waiting until they get home to empty the bladder!!!

But why such extreme vandalism? Is it because students have not been in school for more than one year and they’re making up for lost time? Has social media made it acceptable to record terrible things to share just to get more views?

I’m having a hard time figuring the devious licks trend out. But until the trend dies out, it seems that the students need advocates for them to use the bathroom. Have your child’s medical provider send a letter to allow for bathroom use - and if the bathrooms are locked then they can be allowed use of an open staff bathroom in the office.

If only Sarah Silverman’s doctor had more information about what causes bedwetting. Its not a small bladder! It could have been related to her anxiety and depression that she admits to have contributed. But it could also have been other factors like urine holding or constipation.

Her experience of bedwetting is not unusual. She was embarrassed and felt like no one understood. She puts it into a very relatable and very humorous fashion in her book - “The Bedwetter”.

We frequently recommend using a TENS unit (that can be purchased on Amazon for less than $40) in our pediatric patients who have failed traditional behavior urotherapy and constipation management for their urinary problems like urgency and frequency.

It is a great alternative to those who do not want to used anticholinergics for their symptoms.

Use the TENS 20 minutes at least twice weekly and for at least 3 months. Typical improvement in symptoms are seen after 6-8 weeks of continuous use.

Since our patients have started attending in person school this fall, we have found a problem with bathroom access during school. Students are often prohibited in using the restroom during class or even during passing periods. This leads to urine holding and problems like incontinence and UTIs.

And with the latest TikTok bathroom challenge - DEVIOUS LICKS- where students vandalize school bathrooms, there is even less access to the restroom at schools!

If this is a problem with your child, ask their medical provider to write a letter asking for open access to the restroom.

The sheer number of medications out there to treat constipation can get overwhelming. It’s important to understand the differences between them.

Osmotic laxatives or Stool softeners help to soften the stool so it makes it easier to go. It might cause increased frequency and volume. Expect to see the results within days of starting the medication. So not one to use solely for acute constipation when you need a clean out. Good for daily use.

Stimulant laxatives help to stimulate the gut to move the poop along. It causes a bowel movement within hours for oral meds, and minutes for medication given via the rectum. This is helpful for cases where you need to go quickly! Also great for clean outs!
💩
I often use a combo of both stimulants and stool softeners for clean outs! Sometimes I’ll use a stimulant regularly for a period of time to also reset the system.
